What is a PSA Test?
Prostate-Specific Antigen (PSA) is a protein produced by both normal and malignant cells of the prostate gland. The PSA blood test measures the level of this protein in your blood. It is primarily used to screen for prostate cancer, but also to monitor BPH. An elevated PSA level doesn't automatically mean you have cancer, but it is a vital indicator that further urological investigation is needed.
Decoding Your PSA Results
1. Normal Range: Generally, a PSA level under 4.0 ng/mL is considered normal, though this threshold varies by age (e.g., lower limits for men in their 40s and 50s).
2. Elevated PSA Causes: Prostate cancer is not the only cause. Benign Prostatic Hyperplasia (BPH), urinary tract infections, prostatitis, and even vigorous exercise or ejaculation can raise PSA.
3. PSA Velocity: Urologists look at how fast PSA rises over time. A rapid jump from year to year is often more concerning than a slow, gradual increase due to an enlarging prostate.
4. Free vs. Total PSA: If your total PSA is mildly elevated, a test measuring the ratio of "free" (unbound) PSA can help distinguish between BPH (usually higher free ratio) and cancer.
Next Steps if Your PSA is High
Repeat the Test
Because PSA can fluctuate temporarily, your doctor will likely recommend repeating the blood test after a few weeks, ensuring no recent infections or physical trauma.
Treat Underlying Infections
If prostatitis or a UTI is suspected, a course of antibiotics may be prescribed to see if the PSA levels drop back to normal.
